For all of our residents, the following basic services are available. The services provided will be those the resident wants and those the resident needs, based on the individual’s pre-admission appraisal, and the needs and services plan. Subsequent resident appraisals may result in the need for additional basic services.
We shall provide assistance to residents requesting supportive services such as adult day care centers, entertainment resources, etc., as well as medical, dental or other health related service. Home Health Services may be obtained per Doctor's request.
For all non-emergency medical/dental transportation, we shall assist in transporting or in arranging for a transportation provider. In all cases, we will ensure that the resident's medical and dental needs are met.
To ensure a safe and healthful environment for our clients allowing them to remain at the optimal level of independence with the care and assistance of our trained staff.
You may request a tour of our home at any time. Our staff members are flexible and will assist you at your own convenience.

A face-to-face meeting with us, the owners and staff, to establish our new relationship
You have read the Admission Agreement and agree to all the terms listed.
Have your physician fill out the "Physician's Report for Residential Care Facility for the Elderly" (LIC 602) and TB screening along with a list of medication and medical history.
This form along with several other forms which we will provide are required by the Department of Social Services to which we are governed by.
